There are paramedics on motorcycles and cycles nowadays and ambulances are loaded with equipment as well as air ambulances.  They also need to decide which hospital to go to before they even start off.  You often see ambulances sitting on the roadside for a long time assessing patients before they move off using a siren if they need to.  It's definitely not the same as it used to be.
