As they say it's a MAD, MAD world. Those leaders of countries with significant nuclear weapons know it would be insane to use nuclear weapons in a first strike owing to the nature of mutually assured destruction. Whether the nations(s) being attacked actually respond may be irrelevant as the radioactive fallout, disruption to world trading in food, etc would likely be catastrophic; retaliation would just be the nail in tge coffin of civilisation.

The danger is obviously if states where we consider governments to be unstable either because of religious zealots (Iran?), ideological zealots (N Korea?), or both (didn't a far right Zionist in the Israeli government suggest that?). 

I suppose the real threat is limited use of nuclear weapons - it would cause significant devastation locally and significant chaos in world economic, life support - health, food etc - and other systems resulting in major suffering whereas a MAD catastrophe who be a relatively quick, although painful, end of the human race - and probably most life except the possibly cockroaches.
